Exploring the prospective acceptability of a healthy food incentive program from the perspective of people with type 2 diabetes and experiences of household food insecurity in Alberta, Canada

Objective:
FoodRx is a 12-month healthy food prescription incentive program for people with type 2 diabetes (T2DM) and experiences of household food insecurity. In this study, we aimed to explore potential users’ prospective acceptability (acceptability prior to program use) of the design and delivery of the FoodRx incentive and identify factors influencing prospective acceptability.
Design:We used a qualitative descriptive approach and purposive sampling to recruit individuals who were interested or uninterested in using the FoodRx incentive. Semi-structured interviews were guided by the theoretical framework of acceptability, and corresponding interview transcripts were analysed using differential qualitative analysis guided by the socioecological model.
Setting:Individuals living in Alberta, Canada.
Participants:In total, fifteen adults with T2DM and experiences of household food insecurity.
Results:People who were interested in using the FoodRx incentive (n 10) perceived it to be more acceptable than those who were uninterested (n 5). We identified four themes that captured factors that influenced users’ prospective acceptability: (i) participants’ confidence, views and beliefs of FoodRx design and delivery and its future use (intrapersonal), (ii) the shopping routines and roles of individuals in participants’ social networks (interpersonal), (iii) access to and experience with food retail outlets (community), and (iv) income and food access support to cope with the cost of living (policy).
Conclusion:Future healthy food prescription programs should consider how factors at all levels of the socioecological model influence program acceptability and use these data to inform program design and delivery.

The central argument set out in this Element is that the combination of a perceived radical change in the threat environment post 9/11, and the new capabilities afforded by the long silent reach of the drone, have put pressure on the previously accepted legal frameworks justifying the use of force. This has resulted in disagreements - both articulated and unarticulated - in how the Western allies should respond to both the legal and operational innovations in the use of force that drones have catalysed. The Element focuses on the responses of the UK, France, and Germany to these developments in the context of the changing US approach to the use of force. Locating itself at the interface of international law and politics, this is the first attempt to look at the interplay between technological innovations, legal justifications, and inter-alliance politics in the context of the use of armed drones.

OBJECTIVES/SPECIFIC AIMS: Inflammatory bowel disease (IBD) patients are at an increased risk of Clostridium difficile infection (CDI) but the impact of CDI on disease severity is unclear. The aim of this study was to determine the effect of CDI on long-term disease outcome in a cohort of IBD patients. METHODS/STUDY POPULATION: We analyzed patients enrolled in a prospective IBD natural history registry. Patients who tested positive at least once formed the CDI positive group. We generated a 2:1 propensity matched control cohort based on risk factors of CDI in the year before infection. Healthcare utilization data (emergency department use, subsequent hospitalizations, telephone encounters), medications, labs, disease activity, and quality of life metrics were temporally organized. RESULTS/ANTICIPATED RESULTS: A total of 198 patients (66 CDI, 132 matched controls) were included [56.6% female; 60.1% Crohn’s disease (CD), 39.9% ulcerative colitis (UC)]. Groups were not significantly different in the year before infection in all metrics but in the year of infection, having CDI was significantly associated with more steroid and antibiotic exposure, elevated C-reactive protein or erythrocyte sedimentation rate, and low vitamin D (all p<0.01). Infection was associated with increased disease activity metrics (UC: p=0.036, CD: p=0.003), worse disease-related quality of life (p=0.003), and increased healthcare utilization (p<0.001). In the next year after infection those with prior CDI continued to have increased exposure to vancomycin or fidaxomicin (p<0.001) and all other antibiotics (p=0.01). They also continued to have more clinic visits (p=0.006), telephone encounters (p=0.001), and worse disease-related quality of life (p=0.03), but disease activity and biomarkers of severity were not significantly different between groups. DISCUSSION/SIGNIFICANCE OF IMPACT: CDI infection in IBD is significantly associated with various surrogate markers of disease severity, increased healthcare utilization and poor quality of life during the year of infection. CDI patients continue to experience poor quality of life after infection with increased clinic visits and antibiotic exposure while disease activity is no longer significantly increased. These findings suggest that CDI infection may have a lasting effect on healthcare utilization beyond the acute treatment period.
Attention, vigilance, and visuospatial function in hospitalized elderly medical patients: relationship to delirium syndromal status and motor subtype profile

Background:
The early and effective detection of neurocognitive disorders poses a key diagnostic challenge. We examined performance on common cognitive bedside tests according to differing delirium syndromal status and clinical (motor) subtypes in hospitalized elderly medical inpatients.
Methods:A battery of nine bedside cognitive tests was performed on elderly medical inpatients with DSM-IV delirium, subsyndromal delirium (SSD), and no delirium (ND). Patients with delirium were compared according to clinical (motor) subtypes.
Results:A total of 198 patients (mean age 79.14 ± 8.26) were assessed with full syndromal delirium (FSD: n = 110), SSD (n = 45), and ND (n = 43). Delirium status was not associated with differences in terms of gender distribution, age, or overall medication use. Dementia burden increased with greater delirium status. Overall, the ability to meaningfully engage with the tests varied from 59% for the Vigilance B test to 85% for Spatial Span Forward test and was lowest in patients with FSD, where engagement ranged from 32% for the Vigilance B test to 77% for the Spatial Span Forwards test. The ND group was distinguished from SSD group for the Months of the year backwards, Vigilance B, global VSP, Clock Drawing test, and Interlocking Pentagons test. The SSD group was distinguished from the FSD group by Vigilance A, Spatial Span Forward, and Spatial Span Backwards. Regarding differences among motor subtypes in terms of percentage engagement and performance, the No subtype group had higher ratings across all tests. Delirious patients with no subtype had significantly lower scores on the DRS-R98 than for the other three subtype categories.
Conclusions:Simple bedside tests of attention, vigilance, and visuospatial ability are useful in distinguishing neurocognitive disorders, including SSD from other presentations.

Summary
Schistosomiasis is the generic name given to diseases caused by parasitic blood flukes of the genus Schistosoma. An older name, still widely used in Africa, is bilharzia. Of the three major species that commonly infect humans, two occur predominantly in Africa:
S. mansoni, a cause of intestinal schistosomiasis, also found in Brazil and the Caribbean; and
S. haematobium, the cause of urinary schistosomiasis, also found in the Middle East.
The third major species, S. japonicum, causes another form of intestinal schistosomiasis but is found only in the Far East. A minor species, S. intercalatum, causes infection but insignificant disease in small areas of Central Africa.
Schistosomiasis is typically a chronic infection. Adult worms slowly accumulate from early childhood over a period of 10 to 20 years, and the deposition of eggs in the tissues leads to fibrosis in the intestines and liver (S. mansoni) or the urinary tract (S. haematobium). Mild or moderate symptoms occur in most infected children: severe disease develops in later life in only a minority of these individuals. Isolating the jet in broadband spectra of XBs

Most accretion-powered relativistic jet sources in our Galaxy are transient X-ray binaries (XBs). Efforts to coordinate multiwavelength observations of these objects have improved dramatically over the last decade. Now the challenge is to interpret broadband spectral energy distributions (SEDs) of XBs that are well sampled in both wavelength and time. Here we focus on the evolution of the jet in their broadband spectra. Some of the most densely sampled broadband SEDs of a neutron star transient (IGR J00291+5934) are used to constrain the optically thick–thin break in the jet spectrum. For the black hole transient XTE J1550-564, infrared – X-ray correlations, evolution of broadband spectra and timing signatures indicate that synchrotron emission from the jet likely dominates the X-ray power law at low luminosities (~(2 × 10−4 − 2 × 10−3) LEdd) during the hard state outburst decline.

Technological change in the UK herring industry took place rapidly after 1965, due in part to the active encouragement encouragement given to fishermen to switch from driftnetting to pelagic trawling and purse-seining. The adoption and diffusion of these modern methods of capture stimulated a major expansion of output, but this very success was undermined by the depletion of the fish-stocks on which the industry depended. In the case of the West of Scotland herring fisheries, which were especially important to UK fishermen, the decline in fish-stock biomass caused vessel catch-rates to fall after 1973. The failure of international fisheries management, which acted as a permissive factor in the intensification of fishing effort, also had important economic implications as it resulted in the dissipation of resource-rent. The Authors calculate that the maximum sustainable ‘rent’ which could have been generated from the West of Scotland herring fishery was approximately £14 millions per annum at 1976-equivalent prices.
The resource-rent effectively financed the overcapitalization of the fleet and the decline which followed, and it is the speed with which this occurred that most distinguishes the herring fishery from others where technological change has taken place. The article concludes by arguing that, although the UK public authorities (notably the Herring Industry Board) might reasonably be criticized for pursuing a development strategy which resulted in economic and biological over-fishing, the international regime of fisheries management which prevailed at the time gave them little choice but to adopt a pro-active approach to technical innovation.

Detailed interviews were conducted with 1523 married professional and managerial employees of a major US corporation to test associations of acute and chronic occupational and domestic stress with DSM-III-R major depression and current depressive symptoms. After controlling for demographic and clinical risk factors, both sources of stress were significantly associated with the two measures of depression. On the other hand, neither the demographic and clinical risk factors, nor several psychosocial characteristics (social support, sense of mastery and organizational commitment) moderated the relationship between stress and depression.

We report the results of physical experiments that demonstrate the strong influence of the thermal conductivity of the substrate on the evaporation of a pinned droplet. We show that this behaviour can be captured by a mathematical model including the variation of the saturation concentration with temperature, and hence coupling the problems for the vapour concentration in the atmosphere and the temperature in the liquid and the substrate. Furthermore, we show that including two ad hoc improvements to the model, namely a Newton's law of cooling on the unwetted surface of the substrate and the buoyancy of water vapour in the atmosphere, give excellent quantitative agreement for all of the combinations of liquid and substrate considered.

Does academic achievement in children with epilepsy change over time ?

A 4-year follow-up study of academic achievement in children aged between 11 and 17 years with epilepsy or asthma was carried out to identify differences between the two samples and to identify change in achievement over time. Differences based on sex and seizure severity also were explored. There were 98 subjects in the group with epilepsy and 96 subjects in the group with asthma. Academic achievement in five areas (Composite, Reading, Mathematics, Language, and Vocabulary) was measured using school-administered group test scores. To explore change over time in condition severity, each child was categorized as having a low or high condition severity at baseline (time I) and again 4 years later, resulting in four groups: low–low, low–high, high–low, and high–high. There were too few cases in the low–high group to be included in the analyses. Data were processed using analysis of covariance (ANCOVA), intraclass correlation coefficients, and paired t tests. At follow-up the children with epilepsy continued to perform significantly worse in all five achievement areas than the children with asthma. Children with either inactive or low-severity epilepsy had mean scores comparable to national norms; those with high seizure severity had mean scores ranging from 3 to 5 points below national norms. No changes were found in academic achievement over time for either sample, even among those whose conditions improved. Although boys with high-severity epilepsy continued to have the lowest achievement scores, there was no trend for them to decline in achievement over time.

Taken together, these four volumes comprise the Conflict Series, and represent the fruits of work completed by John Burton, with others, in the last years of his formal academic career in the United States, at the United States Institute of Peace in Washington, DC, and at the Center for Conflict Resolution at George Mason University in Virginia. Burton has now ‘retired’ (though he still writes vigorously) to his native Australia, and that event, together with the appearance of these works, prompts this synoptic evaluation of them in the context of Burton's life and previous work. What makes this particularly interesting in the case of John Burton is that his career has been less than singular; first a civil servant, then a diplomat, then an academic, he moved from Australia, then to the United Kingdom and thence to the United States, with various stops along the way. Though he has written a great deal—books, articles and conference papers—and was a key participant in the organization of the peace research movement in the 1960s, especially the International Peace Research Association and the Conflict Research Society in the United Kingdom (and is described on the back cover of CRP as ‘the founder of the field of conflict resolution’), he was never a professor during his extended residence i n the United Kingdom at, first, University College, London, and then at the University of Kent, achieving that status only later, at George Mason University.

The relationship between the United States and Great Britain is a subject of apparently endless fascination. It has given rise to innumerable books, theses, articles, essays and speculations. It is, however, riven with misunderstandings, inconsistencies and stereotypes. For all that the relationship has been written about, investigated, discussed and analysed, the topic is ripe, even yet, for further discussion.
